Serving Lakewood and Surrounding Communities
As a testament to our longstanding roots, the Foundation’s geographic focus is Lakewood and the surrounding communities served by the former Lakewood Hospital. Grant funding is limited to those organizations that can demonstrate a thoughtful, inclusive, and equitable approach to their work and involvement with individuals and populations considered underserved or impacted by health disparities within this area. These communities and correlating zip codes include Lakewood (44107) and Western Cleveland neighborhoods (44102, 44113, 44109, 44144, 44135, 44111), as well as Rocky River (44116), Fairview Park (44126), North Olmsted (44070), Westlake (44145), and Bay Village (44140).
TAF’s geographic focus encompasses 12 zip codes that represent Western Cleveland neighborhoods and suburbs in Western Cuyahoga County.
The Foundation awards grants to tax-exempt public charities whose efforts are aligned with our mission and funding priority area. Through these grants, we strive to positively impact the health and well-being of people across these communities.
